At Horizon Palliative Vets we offer home consultations for elderly or palliative care patients. We allow the time to go through you pet’s medical history and create a personalised care plan which we send it to you in writing after the appointment. We will also be available on phone and e-mail after the appointment in case things change or if you have any new concerns.

Before your appointment, your Horizon veterinarian will review your pet’s medical history and relevant veterinary records so they can come prepared and make the most of your time together.
During the consultation, your veterinarian will assess your pet’s health, comfort and quality of life, while taking the time to understand your pet, your family and your goals for their care. Together, you’ll discuss your pet’s current needs and what you may expect as their condition progresses.
Following the consultation, we create a personalised care plan tailored to your pet and your family. This includes a clear medication plan, practical guidance for supporting their comfort at home, advice on monitoring pain and quality of life, anticipatory guidance about their condition, and guidance on when to reach out for further support or when it may be time to consider saying goodbye.

Yes. If, during your consultation, you and your Horizon veterinarian feel that euthanasia is the right choice for your pet, it can be provided during the same visit.
Alternatively, if you need more time to consider your options or would prefer to have the consultation first and arrange euthanasia for another day, we can book a separate appointment. Our aim is to support you in making the decision that feels right for your pet and your family, without pressure or expectation.

Yes. Horizon provides at-home palliative care consultations for both dogs and cats across Adelaide, allowing your pet to be assessed in the comfort and familiarity of their own home.

Our at-home appointments are designed to be unhurried, giving you time to talk through your pet’s health, comfort and any concerns or questions you may have.
Palliative Care Consultations are allocated 1.5–2 hours, allowing plenty of time for a thorough assessment and personalised care planning. We allow 1 hour for in-home euthanasia appointments and Quality of Life & End-of-Life Guidance appointments.
Every pet and family is different, so we allow the time needed to ensure you feel supported and your pet’s needs are properly addressed.

Yes. Following a Palliative Care Consultation, you will receive a personalised written care plan tailored to your pet’s condition and your goals for their care.
Your care plan provides clear guidance on medications, monitoring your pet’s comfort and quality of life, practical ways to support them at home, what you may expect as their condition progresses, and when to reach out for further support.
We also provide ongoing support by phone and email, so you don’t have to navigate your pet’s palliative care journey alone.
